Polypharmacy has been recognized as a risk factor for under-prescribing and pretense risks to patients’ safety and well-being. The main objectives of this work were assessment of occurrence, complications and interventions of polypharmacy and irrational use of medicines. A community based survey was conducted in the community pharmacies for a period of 6 months. Patients attending the clinics of practitioners were approached at pharmacy shops and requested to show their prescriptions and by taking prior consent of the patients prescriptions. Prevalence of Polypharmacy and irrational prescriptions were high due to reduced standards of prescribing by physicians. The issue of rational use of medicines was worse. Unnecessary prescribing can be minimized by prescribing specific medication for particular disease.
